Is there a form online for submitting books for pre-screening, with all the info?

There is not an official "Pre-Screen form" but we would ask that you list your comics on one or more inovice of the same tier and note on each copy that it is a Pre-Screen and your minimum grade requirement for the group. (minimum 50 comics) You will be charged for this service after the books have been graded, so you are only charged fully on the books that acutally make the grade. The rest will be charged a lesser screening fee and will be returned to you un-graded.

Is the 50 minimum for pre 75 books or just the moderns?

It is a 50 book minimum for one grade requirement through one tier, either Modern or Economy.

Do the books that don't match the screening requirements (you know, the rejects) get sent back with the slabbed books or separately (i.e., before)?

The rejects are usually shipped back with the graded books unless there are too many of each to safely ship them together.
