Could be too much heat, too much moisture or a myriad of other things done incorrectly.

Really surprised this got the grade bump based on all the new color loss.

Highly doubt it, as it would have been a LOT of CT for CGC to miss on the resto check. Someone screwed the pooch with this press job.

Probably wasn't an experienced presser and like you said, not a mistake Joe would make.  I've heard of this happening when the heat is too high and the substrate between the upper platen and the book basically fuses with the cover.  Especially if they've used the wrong hydration technique and portions of the cover are too damp.  It could be color touch removal or flaking that loosened over time as well, but likely caused by a bush league presser.
