Yes. you have to list the places where your collection is stored.

I use Collectibles Insurance Services.  I have never had to make a claim.  There have been a few threads discussing them.  My impression from those threads is that they are very good about reimbursing your first claim or two, but inclined to drop your coverage if you make more than a couple of claims.  But you may want to track down those threads for more details.
