There are 2 small pieces of tape on the edge of the inside front cover. Would that be considered resto by CGC?

They wouldn't consider this resto as it is conservation, not adding anything to enhance appearance. I've seen many get a blue label with tape noted in comments.

3.5 IMHO Blue Label. But I don't know how CGC would grade the tape. Might grade higher with a green label or conservation label? Maybe 5.0?

Same here with respect to the label and notation.  Regarding the grade, I'd guess a little higher: 4.0 or 4.5.  I had a CGC 4.5 that looked similar to your copy.  Very nice book!
