It looks fake to me.

I think if I was in the business to make fake comic drawings to rip off fans, I might focus on Mignola sketches like these.  The are relatively highly priced, ubiquitous,  somewhat simple, and Mignola has a "slow" line that has a little bit of shake, much like the deliberate lines that a non professional forger would put down (as opposed to quick, bold and sure lines typical of many/most professionals other than Mignola).  Not trying to give anybody ideas, just sayin'....

I completely missed this thread initially and feel I have a few pieces to contribute.  You'll see I have an appreciation for pre-2000 vintage.

Lets start with some Cosmic pieces...

Mike Mignola already confirmed (on facebook) that it's a FAKE.

This seller sold another fake Hellboy a week or 2 back.

This guy has more fakes listed on eBay and people are bidding them up. By looking at this guys completed listings I don't know why anyone would bid on anything he lists. Fake Romita sketches, fake Stan Lee autos, fake Bill Watterson sketches. Yet people are buying and leaving good feedback. Just don't understand.....

And comicartfans.com have many of his listings in their "Hot Auctions" Section. How can whoever is picking those auctions not tell that they are fake. They are only making it worse by featuring the auctions. Anyone knows who runs that at CAF?

You can just contact CAF, Bill Cox will let you know.

I've asked about this before with other forgeries, and CAF hot auctions are automated.  They don't manually choose the auctions.
