This pulp started in 1906 as Railroad Man's Magazine, was merged with Argosy at one point, and is still being published today as Railfan and Railroad.  So there have been a lot of covers over the last 100+ years.  To my knowledge, this is the only horror cover.  If there are others, there's can't be many.

Julie Schwartz got his start by contacting authors he respected and offering to broker rejected stories.  HPL was one of his first clients and it wouldn't surprise me if this was the result of his efforts.
