5.0 seems about right. The horizontal crease shouldn't be a subscription crease... the spine would have crunching if the whole book were folded in half horizontally across. I'd guess it's a production thing.

Right on. It's fairly common on books from certain publishers (Dell comes to mind). CGC will still downgrade for it, but usually not as harshly. And you're right, 5.0 is reasonable...
