Defect in slab 0

The coming is coming in contact with the plastic and creating this unsightly oil spill effect. Now this one got a 9.2 (I, like a complete doofus sent the wrong copy) so I don’t care but I’m about to send my good copy and I don’t want it to look like this. 
 

My questions:

1. Is this preventable?

2. Can a note be made requesting to be checked for this?

I haven't run into the problem with my submissions, but my understanding is that if you get a slab back with excessive NR, CGC will re-slab it
