There are a handful? Of Amazing Spider-Man #5 where the cyan and green plates were switched on pages 13 and 15. You end up with a green spidey and Dr Doom with a red cape.

Does CGC note this as 'qualified or is it just a grader note? Or not noted at all. Any idea how many copies were published before they corrected the error?

Good morning, thanks for your message.  If the mistake only affected a small number of books then we would list it as a manufacturing error and assign it a green qualified label.  The label would state exactly what the book is qualified for.  Generally these types of error only affect an extremely small amount of the print run.

However, If a substantial amount of the print run (hundreds or thousands of books) was affected then CGC would assign the error it's on "Variant name" so to speak and treat the book essentially as if it was intended to be printed that way, complete with a universal label (although Key Comments would still explain what the error is).
