I think ASM #10 (Ditko) had an unpublished cover, along with #220 (Moon Knight), 262 (Photo Cover was published, but there is a line art version by Bob Layton that exists), a few Marvel Superhero Secret Wars issues also have unpublished covers, and last but not least, I believe X-Factor #1 has at least seven different unpublished covers!

ASM 262 unpublished (this unpublished line art cover was completed before the photo cover, and would have been the published cover had the photo cover proved too difficult)
