How to complete the CGC Submission Form

Below is an example of our Submission Form. Please remember to fill it out completely and correctly to avoid any delays.

CGC Submission Form Part 1
1. Enter your name and account number.
2. Select a Tier Service (one item per form). Remember that turnaround times are estimates only!
3. Select any additional services.
4. Complete the shipping information. This is the address CGC will return your books to.
CGC Submission Form Part 2
5. Choose your method of return shipping.
6. Fill in the Quantity, the Date-Month/Year of the COMIC BOOK, Publisher, Title, Issue Number, Certification, Pedigree.Variant, and the Fair Market Value. If you are unsure about this information, check the Indicia.
Don’t forget to total the value of your books.

7. Complete the information to total your submission.
8. This section must be completed fully and signed for credit card payment.
