Captain Marvel's Secret Code - Where is this from?

Came across this Captain Marvel's Secret Code card. Can anyone tell us something about it? Year? How did it come? Is it worth anything?

This was issued by Fawcett comics from about 1941 to 1948 with a pinback button. Also obtained with a dime and a self addressed stamped envelope. The buttons in the 3rd row from the bottom as well as the first one in the second row far left were the 5 versions issued with this card. (Capt. Marvel Shazam)
